Diagnostic Clinic Medical Group is closing.

After more than 50 years of providing care, Diagnostic Clinic Medical Group has made the difficult decision to close our four clinics. Our last day of operation will be August 30, 2024.

Your health and well-being are very important to us, and we understand you may have questions and concerns, especially when it comes to finding a new primary care doctor or specialist. We will be working with you to make sure your care continues after we close.

Finding a new doctor

You will need to work directly with your insurance company to find doctors in your network. You should call the phone number on your insurance card to talk to a representative. Once you have identified a new doctor, you can request medical records via the instructions below.

You, or your new doctor, may request your medical records by following these steps:

 

Making a payment

Continue making your payments for services as you currently do. This includes:

  • Paying online through Healow.
  • Paying through the patient portal.
  • Mailing your check to the address on your billing statement.

After over 50 years of serving our community, DCMG has made the difficult decision to close its doors. Our last day of scheduled patient care will be August 28, 2024, and the official closure date will be August 31. 

The decision to close was driven by the strategy of our parent company, GuideWell, whose core business focuses on delivering the most seamless, high-quality and affordable comprehensive primary care for its Florida Blue health plan members in their group and individual business. These primary care services are provided through Florida Blue | Sanitas Medical Centers. 

Your health has always been our primary focus. We’ll continue to care for our patients and coordinate tests, prescriptions, and referrals until we close. We understand that this may be stressful, especially if you’re managing chronic conditions and other acute care needs. Your DCMG care team will be here to support you through our last day of business.  

We’re grateful for the trust you’ve placed in us over the years, and we’re honored to have been a part of your health journey.  

Our DCMG care team members are exploring employment opportunities at other medical practices in and around the community, and we don’t yet know where they’ll be working once DCMG closes. In the meantime, your insurance company can help you find doctors who are in your network.

We’ll support you as you transition to new care. Your insurance company will help you find local doctors in your network, and we’ll coordinate any referrals, prescriptions and lab work that may be needed by your new care team until we close.

You can call the phone number on your insurance card to talk to a representative or go to your health plan's website to view their provider directories for doctors who are in your plan’s network.

If you want to stay with your primary care doctor or specialist, you’ll need to find out whether your insurance is accepted at their new clinic. Call your new doctor’s office or talk with your insurance company to learn more.

Florida Blue commercial members can receive care at the new Florida Blue | Sanitas Medical Centers opening in or near the current DCMG locations.

All appointments scheduled after DCMG closes will be cancelled. You do not need to cancel these yourself. DCMG team members will handle it.

DCMG will continue to see scheduled patients through August 28. Please call ahead for operating hours, as these will change in the future.  

If you need care after DCMG closes and haven’t found a new primary care doctor yet, you may go to an urgent care facility within your plan’s network, such as GuideWell Emergency Doctors (GEMD). If you’re having a medical emergency, please call 9-1-1 or go to your nearest emergency room.

We’ll support you as you transition to new care. Your insurance company can help you find local doctors who are in your network, and we’ll send over any medical records that are requested to your new provider.  

If you need a new prescription or to refill an existing one, your doctor may order it until DCMG closes. You may continue getting your prescription filled if you have refills available.

If you currently have a referral or authorization for care from your DCMG doctor, you can still use it while you’re under our care. But don’t worry — once you start seeing your new primary care doctor, they’ll help you get any new referrals or authorizations you might need.

Your new primary care doctor will also be able to help with next steps to ensure your continued care for an ongoing health condition.

You or your new primary care doctor may request your medical records by following these steps: 

  • Find the medical record authorization form enclosed with the letter you received from DCMG dated June 18.
  • Visit DC-FL.com to download the authorization form. Complete the form and return it, following the instructions at the bottom.
  • Download your medical records from your patient portal. 
  • Call 1.888.982.2906 and speak to a medical records representative.

After September 1, you can continue making your payments by:

  • Mailing your check to the address on your billing statement. 
  • Paying through your patient portal.

On September 3 there will be four new Florida Blue | Sanitas Medical Centers opening at or near the locations of the former DCMG clinics to serve Florida Blue commercial members. Here’s what’s happening at each site:

  • Largo: A new Florida Blue | Sanitas Medical Center will open near the current DCMG clinic at 1157 Missouri Avenue N, Largo, FL 33770.
  • Countryside: A new Florida Blue | Sanitas Medical Center will open near the current DCMG clinic in a temporary location at 30535-30728 US Highway 19 N, Palm Harbor, FL 34684. In January 2025, the Florida Blue | Sanitas Medical Center will move to a new permanent location in the same shopping center,
  • Crosswinds: A new Florida Blue | Sanitas Medical Center will open in the same location as the current DCMG clinic.
  • Winter Haven: On July 1, DCMG will move to a temporary location at 1020 5th Street SE, Winter Haven, FL 33880, so its current clinic may be remodeled and expanded. After DCMG closes, on September 3 a new Florida Blue | Sanitas Medical Center will open in the same temporary location. In January 2025 the Florida Blue | Sanitas Medical Center will move back into the newly remodeled and expanded clinic, which will have a much larger footprint and include advanced urgent care and an embedded Florida Blue Center.
