After more than 50 years of providing care, Diagnostic Clinic Medical Group has made the difficult decision to close our four clinics.
Finding a new doctor
You will need to work directly with your insurance company to find doctors in your network. You should call the phone number on your insurance card to talk to a representative. Once you have identified a new doctor, you can request medical records via the instructions below.
Getting your medical records
You, or your new doctor, may request your medical records by following these steps:
- Download and complete the authorization form and return via the instructions on the form.
- Download your medical records from your patient portal.
- Call 1-888-982-2906 and speak to a medical records representative.
Making a payment
Continue making your payments for services as you currently do. This includes:
- Paying online through Healow.
- Paying through the patient portal.
- Mailing your check to the address on your billing statement.
